Area of Science:

  • Neuro-oncology
  • Pediatric oncology
  • Clinical trial analysis

Background:

  • Ependymoma treatment involves surgery and radiation, achieving remission in about 50% of newly diagnosed patients.
  • Chemotherapy has a limited role in ependymoma management, primarily used in infants to delay radiation.
  • The efficacy of chemotherapy in older children with newly diagnosed ependymoma is inconclusive, with more data available for recurrent disease.

Purpose of the Study:

  • To analyze clinical trials of single-agent and multiagent chemotherapy in recurrent ependymoma.
  • To discuss the role of chemotherapy in managing recurrent ependymoma in children.
  • To explore future directions in novel medical oncologic therapies for ependymoma.

Main Methods:

  • Review of principal institutional and cooperative group phase I, II, and III clinical trials.
  • Analysis of chemotherapy regimens (single-agent and multiagent) in recurrent ependymoma.
  • Discussion of existing literature and clinical experiences.

Main Results:

  • Chemotherapy has a limited role in ependymoma, particularly in recurrent disease.
  • Data on chemotherapy for recurrent ependymoma is primarily derived from clinical trials in children.
  • The use of chemotherapy in newly diagnosed older children has not shown conclusive benefits.

Conclusions:

  • Surgical resection and radiation therapy remain primary treatments for ependymoma.
  • Chemotherapy's role is limited, especially in recurrent ependymoma, necessitating further research.
  • Novel medical oncologic therapies are being investigated for future ependymoma treatment strategies.
