Background:

  • A roofer experienced work-related respiratory symptoms including cough, shortness of breath, and wheezing.
  • Symptoms exacerbated during sawing of corrugated fiber cement materials.

Observation:

  • Normal baseline spirometry and mild bronchial hyperresponsiveness to carbachol were noted.
  • Skin patch testing for chromium was negative.

Findings:

  • Specific inhalation challenge with fiber cement dust showed a borderline decrease in forced expiratory volume in 1 second (FEV1).
  • Nebulization with potassium dichromate (K2Cr2O7) caused an immediate 20% fall in FEV1 and increased bronchial hyperresponsiveness, indicating occupational asthma.
