Scalp necrosis is a rare but important sign for diagnosing giant cell arteritis (GCA) in elderly patients. Early recognition of this symptom can prevent vision loss associated with GCA.

Area of Science:

  • Ophthalmology
  • Rheumatology
  • Vascular Medicine

Background:

  • Giant cell arteritis (GCA) is a common vasculitis in the elderly.
  • GCA can lead to irreversible vision loss if not diagnosed and treated promptly.
  • Early diagnosis of GCA is crucial for preventing visual complications.

Observation:

  • Scalp necrosis is an uncommon but significant clinical finding in GCA.
  • Two cases of GCA with scalp necrosis are presented, highlighting its diagnostic role.
  • Previous literature lacks reports of scalp necrosis in the ophthalmic context.

Findings:

  • Scalp necrosis was misdiagnosed in one patient before visual loss occurred.
  • Development of scalp necrosis led to the correct diagnosis of GCA in another patient with presumed nonarteritic anterior ischemic optic neuropathy.

Related Experiment Videos

  • A higher incidence of visual loss is associated with GCA presenting with scalp necrosis.
  • Implications:

    • Scalp necrosis should be considered a valuable diagnostic sign for GCA.
    • This sign may indicate a more severe form of vasculitis.
    • Incorporating scalp necrosis into the diagnostic evaluation of suspected GCA is recommended.
