Area of Science:

  • Oncology
  • Radiation Oncology
  • Medical Physics

Background:

  • The optimal radiation dose for prostate cancer eradication is under investigation.
  • Proton therapy offers potential for dose escalation while reducing toxicity.

Purpose of the Study:

  • To evaluate the efficacy and toxicity of proton therapy in localized prostate cancer patients.
  • To assess the impact of initial and post-treatment prostate-specific antigen (PSA) levels on outcomes.

Main Methods:

  • 643 patients with localized prostate cancer received 74-75 CGE proton therapy.
  • Treatment planning utilized a 3D system with dose delivery in fractions.
  • Patients were monitored for treatment response and toxicity.

Main Results:

Related Experiment Videos

  • 5-year disease-free survival was 89%.
  • Disease control rates varied by initial PSA levels, with 100% for PSA < 4.0 ng/ml.
  • Post-treatment PSA nadir < 0.5 ng/ml correlated with 91% 5-year disease-free survival; severe toxicity was < 1%.

Conclusions:

  • Proton therapy at 74-75 CGE demonstrates minimal toxicity and high efficacy, particularly for low-risk prostate cancer.
  • Excellent PSA normalization and disease-free survival were observed.
  • A dose-escalation trial is ongoing to define optimal radiation doses.
