Summary

Gynaecomastia, or male breast enlargement, can be the primary symptom of undiagnosed hyperthyroidism. Prompt treatment of the thyroid condition resolved the gynaecomastia in this case.

  • A 38-year-old Chinese man presented with painful bilateral breast enlargement for one month.
  • Physical examination revealed bilateral tender gynaecomastia, warm peripheries, tachycardia, and goiter.

Findings:

  • Biochemical tests confirmed hyperthyroidism.
  • Treatment of thyrotoxicosis led to the resolution of gynaecomastia.

Implications:

  • This case highlights that gynaecomastia can be the predominant initial symptom of hyperthyroidism.
  • Clinicians should consider hyperthyroidism in men presenting with unexplained gynaecomastia.
