Background:

Many studies suggest a J-shaped or U-shaped relationship between alcohol consumption and mortality.
This observed association may be influenced by various confounding factors rather than solely by alcohol's direct biological effects.

Purpose of the Study:

To critically review methodological issues that may confound the alcohol-mortality relationship.
To explore alternative explanations for observed J-shaped curves in alcohol research.

Main Methods:

This review discusses nine key methodological considerations.
Factors examined include age, sex, cause of death, abstainer characteristics, drinking patterns, smoking, diet, physical activity, social support, personality, and psychopathology.

Main Results:

J-shaped curves may arise from complex interactions between lifestyle, psychosocial factors, and health.
The biological impact of alcohol on mortality might be overestimated due to these confounders.

Conclusions:

The interpretation of alcohol-mortality associations requires careful consideration of numerous confounding variables.
Future research should rigorously account for these methodological issues to clarify the true relationship between alcohol consumption and health outcomes.
