Summary

Bladder angiosarcoma is a rare and often fatal cancer. Multimodality therapy, combining surgery, chemotherapy, and radiotherapy, offers the best chance for survival in patients with this aggressive tumor.

Background:

  • Angiosarcoma of the bladder is an exceedingly rare malignancy.
  • This tumor type has a poorer prognosis compared to angiosarcomas in more common locations.
  • Understanding the unique presentation, treatment, and clinical course is crucial for patient management.

Observation:

  • A review of 10 cases, including a new case, revealed diverse origins, including pre-existing hemangiomas and radiation-induced sarcomas.
  • Hematuria was a consistent presenting symptom across all reviewed cases.
  • While regional lymph nodes were often spared, local recurrence and distant metastasis were common.

Findings:

  • Treatment modalities varied significantly among the reported cases.

  • Eight out of ten patients died within a median follow-up of 23 months, with five succumbing to tumor-related causes (mean survival 10.6 months).
  • The two most recent patients, treated with multimodality approaches, achieved tumor-free survival at 8 and 32 months.
