Background:

Traditional evaluation of contralateral inguinal hernias often involves laparoscopy followed by open groin exploration.
Existing laparoscopic methods for inguinal hernias may require conversion to open surgery.

Purpose of the Study:

To present a purely laparoscopic approach for pediatric inguinal hernias using miniature instruments.
To eliminate the need for groin incisions in the diagnosis and treatment of inguinal hernias.

Main Methods:

A laparoscopic technique was applied to 14 girls (median age, 6.5 years).
Instruments (1.7- or 2-mm) were inserted via the umbilicus for intra-abdominal suturing of open inguinal rings.
Z-sutures were used to close the inguinal rings laparoscopically.

Main Results:

The procedure allows for easy diagnosis of inguinal hernias.
Suturing with miniature instruments requires practice but is effective.
Open processus vaginalis can be sutured laparoscopically if diagnosed during the procedure.
No recurrences or complications were observed in patients with up to 1 year of follow-up.

Conclusions:

A purely laparoscopic approach with miniature instruments integrates diagnosis and treatment for inguinal hernias.
This technique avoids conversion to open surgery, leading to faster recovery and superior cosmetic results.
