Area of Science:

  • Pediatric Gastroenterology
  • Hepatology
  • Infectious Diseases

Context:

  • Cholestatic hepatitis is a less common presentation of acute hepatitis in children.
  • Understanding its characteristics is crucial for diagnosis and management.

Purpose:

  • To determine the incidence, causes, clinical course, and outcomes of cholestatic hepatitis in pediatric patients.
  • To analyze the clinical and biochemical profiles of children with cholestatic hepatitis.

Summary:

  • This retrospective study identified 5 cases (3.45%) of cholestatic hepatitis among 145 hospitalized children with acute hepatitis.
  • Hepatitis A virus (3 cases) and hepatitis B virus (1 case) were identified etiologies, with one case positive for smooth muscle autoantibodies (SMA).
  • Clinical presentation included cholestatic symptoms and elevated liver enzymes; prognosis was generally favorable, resolving within 8 weeks, except for one case with subacute hepatocellular insufficiency.

Impact:

  • Highlights cholestatic hepatitis as an infrequent but significant diagnosis in pediatric acute hepatitis.
  • Provides insights into the viral and autoimmune etiologies of this condition.
  • Emphasizes the generally favorable prognosis, guiding clinical expectations and treatment strategies.
