Summary

Single extraocular muscle hypertrophy often suggests an orbital pseudotumor. However, two cases revealed unexpected malignancy during the diagnostic work-up for this condition.

Background:

Single extraocular muscle hypertrophy is a rare condition.
It is typically associated with orbital pseudotumor, an inflammatory condition of the orbit.

Observation:

Two cases are presented where initial presentation mimicked orbital pseudotumor.
Diagnostic work-up for suspected orbital pseudotumor unexpectedly revealed malignancy.

Findings:

Malignancy should be considered in the differential diagnosis of single extraocular muscle hypertrophy.
This highlights the importance of thorough investigation beyond initial assumptions.

Implications:

Early detection of orbital malignancies can improve patient outcomes.
