Background:

  • Carotid angioplasty with stenting (PTAS) is being explored as an alternative to carotid endarterectomy (CEA).

Purpose of the Study:

  • To compare the safety, cost-effectiveness, and complication rates of PTAS versus CEA.
  • To assess the in-hospital and 30-day risks associated with PTAS.
  • To evaluate non-neurologic complications and overall cost of PTAS compared to CEA.

Main Methods:

  • A review of 107 patients undergoing PTAS was conducted.
  • Safety outcomes, including morbidity and mortality, were analyzed.
  • Cost analysis and investigation of non-neurologic complications were performed.

Main Results:

  • The in-hospital and 30-day risk of stroke and death for PTAS was 9.3%, with a 3.6% major event rate.
  • Minor strokes were significantly more common with PTAS than CEA.
  • PTAS incurred higher non-physician fees ($30,140) compared to CEA ($21,670).

Conclusions:

  • PTAS has not yet demonstrated sufficient advantages to replace CEA.
  • The long-term durability of PTAS requires further investigation.
  • Non-neurologic complications associated with PTAS are a significant consideration.
