Chronic sinusitis (CS) can cause bronchial hyperresponsiveness (BH) in individuals without lower airway disease. Treating CS effectively reduces this BH, improving respiratory health.

Area of Science:

  • Respiratory Medicine
  • Otolaryngology
  • Pulmonology

Background:

  • Chronic sinusitis (CS) may worsen lower airway diseases like asthma and bronchitis.
  • Bronchial hyperresponsiveness (BH) in CS patients without lower airway disease is not well understood.
  • The impact of CS treatment on BH requires further investigation.

Purpose of the Study:

  • To investigate bronchial hyperresponsiveness (BH) in chronic sinusitis (CS) patients without lower airway disease.
  • To compare BH in CS patients with those suffering from stable asthma, chronic bronchitis, or allergic rhinitis.
  • To evaluate the effect of endoscopic sinus surgery on BH in CS patients.

Main Methods:

  • Methacholine (MCh) challenge test was used to assess BH in 42 CS patients, 50 with asthma, 50 with chronic bronchitis, and 40 with allergic rhinitis.

Related Experiment Videos

  • BH was defined as a minimum dose (Dmin) <50 units of MCh, indicating a decrease in respiratory conductance.
  • BH was assessed before and after endoscopic sinus surgery in 7 CS patients.
  • Main Results:

    • 71% of CS patients exhibited BH, irrespective of CS severity, duration, or atopic status.
    • BH prevalence and degree in CS patients were comparable to those with simple chronic bronchitis or allergic rhinitis, but less than in asthma patients.
    • Endoscopic sinus surgery significantly reduced BH (p < 0.01) in CS patients, alongside improvements in nasal symptoms and sinus lesions.

    Conclusions:

    • Chronic sinusitis itself induces bronchial hyperresponsiveness (BH) to a degree similar to simple chronic bronchitis and allergic rhinitis.
    • BH in CS patients occurs independently of clinical background factors.
    • Effective treatment of CS, including surgery, can significantly decrease BH.
