Summary

Patients with idiopathic bile acid malabsorption do not show increased dihydroxy bile acids, suggesting this is not the cause of their chronic diarrhoea. Their risk of gallstones also remains unchanged.

Background:

  • Chronic diarrhoea is a key symptom of idiopathic bile acid malabsorption (IBAM).
  • The exact cause of diarrhoea in IBAM is unknown, particularly whether specific bile acids contribute.
  • Impact of IBAM on bile composition, including cholesterol saturation, is not well understood.

Purpose of the Study:

  • To determine if dihydroxy bile acids enrich the bile acid pool in IBAM patients.
  • To investigate if cholesterol supersaturation of duodenal bile occurs in IBAM due to bile acid depletion.

Main Methods:

  • Studied 13 patients with diagnosed IBAM and 23 controls.
  • Collected and analyzed duodenal bile fluid composition, including lipids and bile acids.
  • Utilized 75SeHCAT test for IBAM diagnosis and high-performance liquid chromatography for bile acid analysis.

Main Results:

  • Patients with IBAM had similar cholesterol saturation indices compared to controls.
  • No significant enrichment of dihydroxy bile acids was found in IBAM patients.
  • A decrease in cholic acid percentage was observed in IBAM patients.

Conclusions:

  • The mechanism of diarrhoea in IBAM does not appear to involve enrichment with dihydroxy bile acids.
  • IBAM patients do not face an elevated risk of developing cholesterol gallstones.
