Summary

Pigmentary glaucoma eyes do not show unique optic disc morphology compared to primary open angle glaucoma, despite higher intraocular pressure. This suggests optic nerve head appearance is not a key differentiator in pigmentary glaucoma.

Background:

  • Pigmentary glaucoma is characterized by pigment dispersion syndrome.
  • Anterior segment morphology in pigmentary glaucoma is distinct.
  • Optic nerve head (ONH) morphology in pigmentary glaucoma requires further investigation.

Purpose of the Study:

  • To compare the ONH morphology between eyes with pigmentary glaucoma and primary open angle glaucoma (POAG).
  • To determine if pigmentary glaucoma exhibits a pathognomonic ONH morphology.

Main Methods:

  • Morphometric evaluation of optic disc photographs from 62 pigmentary glaucoma patients and 566 POAG patients.
  • Comparison of ONH parameters including optic disc size and shape, neuroretinal rim, optic cup depth, parapapillary atrophy zones, retinal vessel diameter, disc hemorrhages, and retinal nerve fiber layer defects.

    • The beta zone of parapapillary atrophy was slightly smaller in the pigmentary glaucoma group, though not statistically significant (p=0.06).
    • Mean maximal intraocular pressure and intraocular pressure amplitude were significantly higher in the pigmentary glaucoma group (p<0.001).

    Conclusions:

    • Eyes with pigmentary glaucoma do not display a pathognomonic ONH or retinal nerve fiber layer morphology compared to POAG.
    • Despite higher intraocular pressure peaks and amplitude in pigmentary glaucoma, ONH morphology remains similar to POAG.
    • The slightly smaller beta zone in pigmentary glaucoma may be associated with elevated intraocular pressure.
