    Summary

    Selective renin sampling pinpointed a high-renin source in a teenager’s left kidney after trauma. Surgical removal of the affected tissue successfully corrected his hypertension, demonstrating a new diagnostic approach.

    Background:

    Observation:

    • A 16-year-old male presented with gradually developing hypertension following blunt left renal trauma.
    • Selective renin sampling from renal vein tributaries revealed a localized high-renin source in the lower pole of the left kidney.
    • Renin secretion from the affected ischemic kidney pole suppressed secretion from the contralateral kidney and the normal portion of the affected kidney.

    Findings:

    • The localized renin secretion from the ischemic kidney pole was the cause of the patient's hypertension.
