Perihepatitis associated with salpingitis in adolescents

I F Litt, M I Cohen

    JAMA
    |September 15, 1978
    PubMed
    Summary

Gonococcal perihepatitis, a complication of gonorrhea, affects nearly a third of adolescent salpingitis patients. Prompt penicillin treatment resolves liver dysfunction, suggesting this condition is underdiagnosed in adolescents.

    Area of Science:

    • Gynecology
    • Hepatology
    • Infectious Diseases

    Background:

    • Perihepatitis, a complication of gonorrhea, is often underdiagnosed.
    • Adolescent salpingitis presents unique diagnostic challenges.

    Purpose of the Study:

    • To determine the incidence of gonococcal perihepatitis in adolescents.
    • To evaluate the impact of gonococcal perihepatitis on liver function tests.
    • To assess treatment outcomes for this condition in adolescents.

    Main Methods:

    • Retrospective review of 137 adolescent salpingitis cases.
    • Clinical examination for right upper quadrant tenderness and hepatic enlargement.
    • Analysis of liver function tests, including amino transferase levels.

    Main Results:

    • Twenty-seven percent of adolescents with salpingitis exhibited signs of perihepatitis.
    • Hepatitis was indicated by right upper quadrant tenderness or hepatic enlargement.
    • Nineteen of 59 tested adolescents showed elevated amino transferase levels.
    • All symptoms resolved rapidly with intravenous penicillin therapy.

    Conclusions:

    • Gonococcal perihepatitis may be more common in adolescents than previously reported.
    • Perihepatitis should be considered in adolescent salpingitis evaluations.
    • Early diagnosis and treatment with penicillin are effective.
