Summary

Dual sphincterotomy (biliary and pancreatic) is not always necessary after pancreatic sphincterotomy for chronic pancreatitis. It is only indicated for patients with both dilated bile ducts and elevated alkaline phosphatase, reducing cholangitis risk.
