Background:

Acute myelogenous leukemia (AML) and myelodysplastic syndrome (MDS) are hematologic malignancies.
Cytarabine (Ara-C) is a common chemotherapeutic agent used in treating these conditions.
Pericarditis is inflammation of the pericardium, potentially leading to serious complications.

Observation:

Two patients with AML/MDS developed fever and pericardial effusion during or after cytarabine (Ara-C) treatment.
Diagnostic pericardiocentesis revealed exudative effusion with neutrophils, ruling out infection.
Symptoms resolved with corticosteroids and pericardial drainage.

Findings:

The cases suggest a diagnosis of allergic pericarditis induced by cytarabine (Ara-C).
Six additional patients with Ara-C syndrome were encountered, indicating a potential pattern.
