Bile acid replacement therapy improved fat absorption and weight in a short-bowel syndrome patient. This treatment for fat malabsorption did not worsen diarrhea, challenging previous beliefs.
Area of Science:
- Gastroenterology
- Metabolic Disorders
- Nutritional Science
Background:
- Short-bowel syndrome (SBS) causes fat malabsorption due to reduced bile acid secretion.
- Bile acid replacement therapy is avoided in SBS due to concerns of exacerbating diarrhea.
- This study investigated conjugated bile acid therapy in an SBS patient with an ileostomy.
Observation:
- The patient had severe malabsorption and was dependent on parenteral nutrition.
- Metabolic balance studies and a 4-month outpatient trial were conducted.
- Two types of conjugated bile acids were tested: natural ox bile mixture and synthetic cholylsarcosine.
Findings:
- Both bile acid preparations significantly increased fat absorption by approximately 40 g/day.
- Calcium absorption also improved with bile acid therapy.
- Neither therapy led to a significant increase in ileostomy output; the patient gained weight without adverse effects.
Implications:
- Conjugated bile acid replacement therapy can be beneficial for selected SBS patients.
- This challenges the contraindication of bile acid therapy in SBS.
- It offers a potential treatment to improve nutrient absorption and weight in SBS.
