Summary

This case report details a young man with anaplastic large cell lymphoma (ALCL) of the null-cell type, presenting with severe neurological deficits due to spinal involvement. Despite aggressive treatment, the patient experienced bone marrow relapse and succumbed to the disease.

Background:

  • Anaplastic large cell lymphoma (ALCL) is a distinct type of non-Hodgkin lymphoma.
  • Null-cell type ALCL is characterized by specific immunophenotypic markers.
  • Bone involvement in ALCL can lead to significant clinical complications.

Observation:

  • A 21-year-old male presented with incomplete paraplegia and incontinence.
  • Imaging revealed extensive bone lesions and para-aortic lymphadenopathy.
  • Lumbar spine biopsy showed a proliferation of CD30+, EMA+, vimentin+, and p80NPM/ALK+ anaplastic large cells.

Findings:

  • The patient was diagnosed with null-cell type ALCL with widespread bone metastasis.
  • Immunohistochemistry confirmed the ALCL phenotype, excluding Epstein-Barr virus association.
  • Despite multi-modal therapy including chemotherapy, radiation, and stem cell transplant, bone marrow infiltration occurred.

Implications:

  • This case highlights the aggressive nature of null-cell type ALCL, even in young patients.
  • Rapid progression and treatment resistance underscore the challenges in managing disseminated ALCL.
  • Further research into novel therapeutic strategies for refractory ALCL is warranted.
