Background:

  • Primary liposarcoma of the urinary bladder is an exceptionally rare malignancy.
  • Round cell liposarcoma is an aggressive subtype with limited reported cases in the bladder.

Observation:

  • An 80-year-old female presented with hematuria due to a large (6.5 cm), solid, non-circumscribed bladder tumor.
  • Radiological, histological, and immunohistochemical analyses were performed.

Findings:

  • Histology revealed isolated lipoblasts amidst small round cells positive for S-100 and vimentin.
  • A high proliferation rate was indicated by Ki-67 staining.
  • The patient succumbed to the disease 10 months post-diagnosis.

Implications:

  • This case underscores the rarity and poor prognosis associated with primary bladder liposarcoma.
