Summary

Ovarian cysts can accumulate methotrexate, leading to higher local drug concentrations. Doses under 100 mg/m2 may be safe for small cysts, but large cysts risk toxicity with high methotrexate doses.

Background:

Observation:

  • Intraoperative serum and ovarian cyst methotrexate levels were measured.
  • Pharmacokinetic analysis was performed using specialized software.

Findings:

  • Methotrexate concentration in an ovarian cyst (3.1 x 10(-7) M) was higher than in serum (1.6 x 10(-7) M).
  • This indicates ovarian cysts act as a methotrexate depot.

Implications:

  • Methotrexate doses <100 mg/m2 may be safe for patients with small ovarian cysts.
  • High-dose methotrexate with large ovarian cysts may cause local and systemic toxicity.
