Background:

  • Gluten sensitive enteropathy (GSE) is linked to liver abnormalities like primary biliary cirrhosis and chronic active hepatitis.
  • The co-occurrence of GSE and liver disease in children warrants further investigation.

Purpose of the Study:

  • To investigate the prevalence of gluten sensitive enteropathy in children with unexplained liver abnormalities.
  • To assess the impact of a gluten-free diet on liver function and intestinal health in these patients.

Main Methods:

  • Nine children with short stature, chronic diarrhea, or unexplained hepatosplenomegaly underwent duodenal biopsy.
  • Serum aminotransferase levels were abnormal in all patients; other liver damage etiologies were excluded.
  • Six patients were tested for gliadin antibodies; five underwent liver biopsy.

Main Results:

  • Intestinal biopsies confirmed gluten sensitive enteropathy in all nine children.
  • All patients had positive IgG gliadin antibodies; three had positive IgA gliadin antibodies.
  • A gluten-free diet normalized transaminase levels within 3 months in seven patients, with intestinal mucosa returning to normal after one year.

Conclusions:

  • Gluten sensitive enteropathy should be considered in children with elevated serum transaminases or cryptogenic liver disease.
  • Early diagnosis and dietary intervention can significantly improve liver function and intestinal health in affected children.
