This study reports a case of respiratory bronchiolitis-associated interstitial lung disease (RB-ILD) in a long-term smoker. Diagnosis was confirmed via thoracoscopic lung biopsy, highlighting RB-ILD as a cause of interstitial lung disease.

Background:

  • Interstitial lung diseases (ILDs) encompass a diverse group of lung disorders.
  • Smoking is a known risk factor for several ILDs, including respiratory bronchiolitis-associated interstitial lung disease (RB-ILD).

Observation:

  • A 48-year-old male smoker presented with chronic dry cough and clubbing.
  • Chest imaging revealed diffuse reticulonodular opacities and specific patterns suggestive of interstitial lung disease.
  • Initial transbronchial biopsy was inconclusive, necessitating a thoracoscopic approach.

  • Thoracoscopic lung biopsy confirmed respiratory bronchiolitis with mononuclear inflammation and interstitial thickening.
