This case study highlights a rare presentation of miliary tuberculosis associated with a subcutaneous tuberculous abscess. Prompt diagnosis and treatment led to the resolution of severe symptoms, including respiratory failure.

Background:

  • Miliary tuberculosis (TB) is a severe disseminated form of TB.
  • Subcutaneous tuberculous abscesses are uncommon manifestations of TB.

Observation:

  • A 62-year-old woman presented with fever, lymphadenopathy, and pulmonary nodules, diagnosed as miliary TB.
  • A concurrent subcutaneous tumor on her thigh was identified as a tuberculous abscess.

Findings:

  • The patient experienced acute respiratory failure and disseminated intravascular coagulation, which resolved with anti-TB therapy.
  • Drainage of the subcutaneous abscess led to significant improvement in fever and pulmonary findings.

Implications:

  • This case underscores the importance of considering unusual presentations of TB.
