    Summary

    Ectopic ossification after hip replacement occurred in 37-38% of patients. While slight to moderate ossification did not cause pain or affect function, it did reduce hip mobility, with significant impact observed in higher grades.

    Background:

    • Ectopic ossification (EO) is a potential complication following hip replacement surgery.
    • Understanding the incidence and impact of EO is crucial for patient outcomes.

    Purpose of the Study:

    • To determine the incidence of radiographically visible ectopic ossification around the hip joint after endoprosthetic replacement.
    • To correlate the extent of EO with pain, function, and mobility.

    Main Methods:

    • 145 hip joints in 132 patients undergoing endoprosthetic replacement (total hip or femoral head replacement) were analyzed.
    • An antero-lateral surgical approach without detachment of the greater trochanter was used in all cases.
    • Patients were followed for 3, 6, and 12 months post-operation, with EO graded (0-III) and correlated with clinical outcomes.

    Main Results:

    • Ectopic ossification was observed in 37% of total hip replacements and 38% of femoral head replacements.
    • 95% of EO was evident within 3 months, with no increase in size but potential density increase over time.
    • Slight to moderate EO (grades I-II) did not cause pain or affect hip function but significantly reduced hip mobility (p<0.05 for grade I vs II, p<0.001 for grade 0 vs II).

    Conclusions:

    • Ectopic ossification is a common finding after hip endoprosthetic replacement.
    • While not typically causing pain or functional impairment, EO can significantly limit hip mobility.
    • The degree of mobility reduction is directly related to the extent of ectopic ossification.
