Correlation between electrocardiographic changes, serum digoxin, and total body digoxin content

P Joubert, B Kroening, M Weintraub

    Summary

This study found a linear relationship between serum digoxin levels and the PTQ index, which assesses electrocardiogram changes. This correlation improved when considering the total body burden of digoxin.

    Area of Science:

    • Cardiology
    • Pharmacology
    • Biomedical Engineering

    Background:

    • Digoxin is a cardiac glycoside used to treat heart failure and arrhythmias.
    • Monitoring serum digoxin levels is crucial for therapeutic efficacy and toxicity.
    • Electrocardiogram (ECG) changes can indicate digoxin toxicity.

    Purpose of the Study:

    • To investigate the relationship between serum digoxin levels and specific ECG parameters.
    • To evaluate the utility of the PTQ index in reflecting digoxin's effects.
    • To determine if total body burden of digoxin provides a better correlation with ECG changes.

    Main Methods:

    • Four healthy volunteers received escalating doses of digoxin (0.25 mg then 0.5 mg daily).
    • Serial electrocardiograms (ECGs) and serum digoxin levels were measured.

    Related Experiment Videos

  • A PTQ index (based on PR-interval, corrected QT-time, T-wave depression) was calculated.
  • Correlation analysis was performed between serum digoxin levels, PTQ index, and computer-calculated total body burden of digoxin.
  • Main Results:

    • A linear relationship was observed between serum digoxin levels and the PTQ index.
    • This correlation was statistically significant in 3 out of 4 subjects.
    • The correlation between the PTQ index and the total body burden of digoxin was stronger.

    Conclusions:

    • The PTQ index may serve as a useful indicator of serum digoxin levels and their ECG effects.
    • Total body burden of digoxin may be a more accurate predictor of ECG changes than serum concentration alone.
    • Further research with larger cohorts is warranted to validate these findings.
