Area of Science:

  • * Infectious Diseases
  • * Urology
  • * Transplant Medicine

Background:

  • * Vancomycin-resistant enterococci (VRE) are a growing concern in healthcare settings.
  • * Prostatitis is an inflammation of the prostate gland, often caused by bacterial infections.
  • * This case represents the first reported instance of prostatitis specifically caused by VRE.

Observation:

  • * A 42-year-old liver transplant recipient presented with chronic prostatitis.
  • * The causative agent was identified as Enterococcus faecium, exhibiting resistance to vancomycin, ampicillin, ciprofloxacin, and doxycycline.
  • * Standard treatment options were limited due to the extensive antibiotic resistance.

Findings:

  • * Treatment with a combination of rifampin and nitrofurantoin for six weeks led to a long-lasting cure.

Related Experiment Videos

  • * In vitro susceptibility testing guided the selection of effective antimicrobial agents.
  • * Other agents like quinupristin/dalfopristin and chloramphenicol were deemed unlikely to reach adequate prostatic tissue concentrations.
  • Implications:

    • * This case highlights the potential for VRE to cause prostatitis, even in immunocompromised patients.
    • * Combination therapy with rifampin and nitrofurantoin presents a viable treatment strategy for VRE prostatitis.
    • * Further research is needed to establish optimal treatment protocols and understand pharmacokinetic challenges in prostatic infections.
