Background:

Kyphoscoliosis is a known complication of certain bone dysplasias.
Cleidocranial dysplasia is one such condition associated with kyphoscoliosis.

Purpose of the Study:

To report a distinct skeletal dysplasia characterized by defective ossification of the ischial rami, severe kyphoscoliosis, and normal clavicles.
To highlight the importance of early recognition for preventing complications associated with this syndrome.

Main Methods:

Case series analysis of eight patients aged 1 day to 33 years.
Evaluation included radiological findings, family history, and long-term follow-up (5-30 years).

Main Results:

All patients exhibited severe, early-onset, rapidly progressing thoracic scoliosis, with spinal cord compression and respiratory issues in four.

Bilateral, symmetrical incomplete ossification of the ischial rami was consistently observed.

Peculiar facies with retrognathia and normal clavicles were noted. Three patients were from the same family, suggesting a genetic link.

Conclusions:

Ischio-vertebral dysplasia is proposed as a distinct entity, differentiated from cleidocranial dysostosis by specific radiological and genetic findings.
The combination of kyphoscoliosis and pelvic abnormalities is pathognomonic for this condition.
Early detection and intervention can prevent severe neurological and respiratory complications.
