Background:

  • A normal electrohepatogram (EHG) in a canine model consists of monophasic, positively deflected slow waves (pacesetter potentials or PPs).
  • Dysrhythmic EHG patterns were observed in canines following liver insults like vessel clamping or irradiation.
  • Electrohepatography holds potential as an investigative tool for human liver diseases.

Purpose of the Study:

  • To develop and characterize a percutaneous electrohepatogram (EHG) in humans.
  • To assess the feasibility of non-invasive EHG recording for liver disease diagnosis.

Main Methods:

  • Twenty-three healthy volunteers and 13 patients with ventral hernias underwent EHG recording during surgery.
  • Three electrodes were sutured to the liver capsule and later placed percutaneously on the skin.
  • Optimal electrode placement for percutaneous recording was identified 1.5-2 cm below the costal margin.

Main Results:

  • Monophasic, positively deflected PPs were recorded both directly from the liver and percutaneously.
  • The percutaneous EHG in humans exhibited a mean frequency of 8.3 cycle/s and amplitude of 56.5 microV.
  • Percutaneously recorded waves were identical to those recorded directly from the liver.

Conclusions:

  • A normal percutaneous electrohepatogram (EHG) in humans can be characterized.
  • This non-invasive method shows promise for detecting changes associated with liver diseases.
  • Percutaneous EHG may serve as a valuable diagnostic tool for various hepatic conditions.
