    Summary

    Delayed gastric emptying after gastric resection is more common with vagotomy and hemigastrectomy than subtotal gastrectomy. Conservative management is recommended unless obstruction is suspected.

    Background:

    • Gastric resection is a common procedure for various gastrointestinal conditions.
    • Delayed gastric emptying (DGE) is a significant post-operative complication following gastric surgery.
    • Understanding factors influencing DGE is crucial for patient recovery and management.

    Observation:

    • A review of 46 patients unable to tolerate a solid diet within two weeks post-gastric resection, excluding other complications.
    • The study compared the incidence and duration of DGE in patients undergoing different types of gastric resection.
    • Mechanical factors were identified as the cause of delay in only 10% of cases.

    Findings:

    • Patients with vagotomy and hemigastrectomy experienced delayed gastric emptying 2.5 times more frequently than those with subtotal gastrectomy.
    • The duration of post-operative delay was often prolonged in the vagotomy and hemigastrectomy group.
    • Localized starch peritonitis is proposed as a potential explanation for functional efferent limb ileus.

    Implications:

    • The findings suggest that surgical approach significantly impacts the risk of DGE.
    • Conservative management is advised for most cases of DGE, reserving reoperation for severe or obstructive cases.
    • Further investigation into "functional efferent limb ileus" and its management is warranted.
