Insights
Contrast-enhanced ultrasound detected neovascularization in carotid artery plaque, indicating inflammation and potential vulnerability. This imaging technique may help assess stroke risk in patients with internal carotid artery narrowing.
Area of Science:
- Vascular Ultrasound
- Cerebrovascular Disease
- Atherosclerosis Imaging
Background:
- Transient ischemic attacks (TIAs) are often caused by vulnerable atherosclerotic plaques in the internal carotid artery (ICA).
- Assessing plaque vulnerability is crucial for stroke risk stratification.
- Neovascularization within atherosclerotic plaques is linked to inflammation and progression.
Observation:
- A 62-year-old male presented with recurrent TIAs.
- Ultrasound revealed 70% stenosis of the right proximal ICA.
- Contrast-enhanced ultrasound suggested neovascularization within the carotid plaque.
Findings:
- Carotid endarterectomy specimen confirmed extensive plaque neovascularization.
- Immunohistochemistry showed dense macrophage infiltration correlating with neovascularization.
- Plaque neovascularization is associated with inflammation and progression.
Implications:
- Contrast-enhanced ultrasound may identify vulnerable carotid plaques.
- This technique could offer additional evidence for plaque instability.
- Further research is needed to establish the clinical utility of contrast-enhanced ultrasound for plaque vulnerability assessment.
Abstract:
A 62-year-old man was admitted after recurrent transient left-sided weakness and sensory loss. Ultrasound (US) examination revealed a 70% narrowing of the right proximal internal carotid artery (ICA). Contrast-enhanced US suggested plaque neovascularization (figure, A). Carotid endarterectomy of the right ICA was performed. Immunohistochemistry of the specimen showed, corresponding to the US findings, extensive plaque neovascularization associated with dense macrophage infiltration (figure, B, C). Plaque neovascularization is associated with inflammation and plaque progression.(1) The detection of plaque neovascularization by contrast-enhanced US could give further evidence of plaque vulnerability, but further study is needed to determine its value.
