Insights
Image analysis of Ki-67 expression in follicular lymphoma (FL) offers a reproducible method. High Ki-67 levels correlate with shorter survival, suggesting its prognostic value in FL treatment.
Area of Science:
- Oncology
- Pathology
- Medical Imaging
Background:
- Ki-67 is a key proliferation marker in lymphomas.
- Manual Ki-67 counting in immunohistochemistry (IHC) is subjective and time-consuming.
- Image analysis offers automated quantification for IHC.
Purpose of the Study:
- To evaluate Ki-67 expression using image analysis in follicular lymphoma (FL).
- To determine the prognostic significance of Ki-67 in FL patients treated with rituximab-containing regimens.
- To assess the correlation between Ki-67 expression and FL grade.
Main Methods:
- Analysis of 82 newly-diagnosed FL patients treated with rituximab-containing regimens.
- Quantification of Ki-67 positive cells using image analysis software.
- Comparison of Ki-67 expression across different FL grades and correlation with overall survival.
Main Results:
- Median Ki-67 expression was 17.0%.
- High Ki-67 expression showed a trend towards shorter overall survival (P = 0.058).
- FL grade 1-2 exhibited significantly lower Ki-67 expression than FL grade 3a.
Conclusions:
- Image analysis provides an accurate, reproducible, and efficient method for measuring Ki-67 in FL.
- Ki-67 expression, quantified by image analysis, may serve as a prognostic marker for treatment selection and outcome prediction in FL.
Abstract:
Ki-67 is a useful proliferation marker in various tumors including lymphoma. In general, the number of Ki-67 positive cells in immunohistochemistry (IHC) is counted manually for routine pathological diagnosis. However, a manual count is subjective and time consuming. Currently, image analysis is often used for the quantification of positive cells in tissue in IHC. Thus, to determine the pathological prognostic factors for follicular lymphoma (FL), we studied the relationship between Ki-67 expression in IHC and the treatment effect and prognosis using image analysis software. We analyzed 82 newly-diagnosed patients with FL. All patients were treated with rituximab-containing regimens. The median Ki-67 expression was 17.0%. A high expression of Ki-67 tended to be associated with short overall survival (P = 0.058). Moreover, Ki-67 expression was significantly lower in patients with FL grade 1-2 than in those with FL grade 3a. This study suggests that image analysis provides an accurate, reproducible, and easy method of measuring Ki-67 expression in IHC in FL, and is possibly a useful marker for treatment selection or prognosis prediction in FL.
