Resumen

Sus antecedentes:

  • Un metanálisis indicó que la suplementación con calcio previene efectivamente la preeclampsia.
  • El ensayo de Calcio para la Prevención de la Preeclampsia (CPEP) del NIH no encontró reducción de riesgo en mujeres nulíparas sanas.
  • Las discrepancias entre los estudios requieren una mayor investigación sobre el papel del calcio en la prevención de la preeclampsia.

Objetivo del estudio:

  • Para conciliar hallazgos contradictorios entre un metanálisis y el ensayo CPEP.
  • Evaluar el impacto de la heterogeneidad de los efectos en las discrepancias de los estudios.
  • Evaluar la eficacia de la suplementación con calcio para la prevención de la preeclampsia.

Principales métodos:

  • Revisión sistemática de la literatura de artículos en inglés publicados antes del 10 de julio de 1997.
  • Inclusión de ensayos que reportan datos sobre la preeclampsia y la suplementación con calcio.
  • Análisis de 14 ensayos, estratificados por grupos controlados con placebo y poblaciones de riesgo (alto riesgo vs. bajo riesgo).

Principales resultados:

  • Se observó una heterogeneidad sustancial entre los ensayos (P = .001).
  • En ensayos controlados con placebo con poblaciones de bajo riesgo, el calcio no mostró ningún beneficio significativo (RR, 0.79; P = .30), alineándose con los resultados del ensayo CPEP.
  • En poblaciones sanas de alto riesgo, el calcio sugirió un fuerte beneficio (RR, 0,19; P = 0,001), aunque los resultados fueron equívocos debido al pequeño tamaño de la muestra (n = 225) y los datos inconsistentes.

Conclusiones:

  • La eficacia de la suplementación con calcio para la prevención de la preeclampsia en poblaciones sanas de alto riesgo requiere más investigación.
  • Una sola medida resumida es insuficiente para los metanálisis con una heterogeneidad significativa.
  • La identificación de covariables para reducir la heterogeneidad es crucial para optimizar las estrategias de tratamiento.
