The placebo effect occurs when people's expectations or beliefs influence or determine their experience in a given situation. In other words, simply expecting something to happen can actually make it happen.

Randomized Experiments

The randomization process involves assigning study participants randomly to experimental or control groups based on their probability of being equally assigned. Randomization is meant to eliminate selection bias and balance known and unknown confounding factors so that the control group is similar to the treatment group as much as possible. Blinding

Blinding is a commonly used method of not telling participants which treatment a subject is receiving. Blinding is a critical part of a randomized control trial or RCT. It reduces the bias that affects the results. In an RCT, blinding is used in the form of a placebo. A placebo effect occurs when untreated subjects falsely believe they have received the treatment and report improved symptoms. A placebo or a dummy treatment is administered to subjects to negate the bias caused by such an effect.

The hazard ratio (HR) is a widely used measure in clinical trials to compare the risk of events, such as death or disease recurrence, between two groups over time. It reflects the ratio of hazard rates—the instantaneous risk of the event occurring—between a treatment group and a control group. Sus antecedentes:

  • La orbitopatía de Graves es una condición autolimitante con un tratamiento óptimo incierto para la gravedad moderada.
  • Las opciones actuales de tratamiento incluyen esteroides, irradiación o terapia combinada.
  • La eficacia de la irradiación de haz externo frente a la irradiación simulada para esta condición requiere investigación.

Objetivo del estudio:

  • Evaluar la eficacia de la irradiación externa en comparación con la irradiación simulada en pacientes con orbitopatía de Graves moderadamente severa.
  • Para evaluar los resultados del tratamiento utilizando medidas cualitativas y cuantitativas.

Principales métodos:

  • Se realizó un ensayo clínico aleatorizado doble ciego con 60 pacientes.
  • Los pacientes recibieron radioterapia (20 Gy en diez fracciones) o irradiación simulada.
  • Los resultados se evaluaron a las 24 semanas utilizando criterios mayores/menores, apertura de los párpados, proptosis, movimientos oculares y puntajes de actividad clínica.

Principales resultados:

  • Los resultados cualitativos fueron exitosos en el 60% de los pacientes irradiados frente al 31% de los pacientes irradiados simulados (p=0,04).
  • Se observaron mejoras en la diplopía y la elevación del ojo, pero no en la proptosis ni en la hinchazón de los párpados.
  • Los campos binoculares de visión única mejoraron en el grupo de radioterapia, aunque el 25% aún necesitaba cirugía de estrabismo.

Conclusiones:

  • La radioterapia demostró eficacia en la mejora de ciertos aspectos de la orbitopatía de Graves moderadamente grave.
  • El principal beneficio de la radioterapia parece estar en el tratamiento de la alteración de la motilidad.
  • Su uso debe considerarse específicamente para problemas de motilidad en la orbitopatía de Graves.
