Sus antecedentes:

  • Se sabe que las unidades de ictus mejoran los resultados de los pacientes, reduciendo la mortalidad y la dependencia.
  • Sin embargo, las diferencias específicas de manejo y su impacto en la recuperación del paciente siguen sin estar claras.

Objetivo del estudio:

  • Para comparar el manejo y las complicaciones de los pacientes con accidente cerebrovascular agudo ingresados en una unidad de accidente cerebrovascular frente a una sala general.
  • Identificar los factores que contribuyen a los mejores resultados observados en las unidades de ictus.

Principales métodos:

  • Un ensayo aleatorizado que involucró a 304 pacientes asignados a unidades de accidente cerebrovascular o salas generales con apoyo especializado.
  • Recopilación de datos prospectivos sobre intervenciones y complicaciones durante 3 meses, evaluados por observadores independientes.
  • Resultado medido usando el puntaje modificado de Rankin a los 3 meses.

Principales resultados:

  • Los pacientes de la unidad de accidente cerebrovascular recibieron un monitoreo más frecuente, oxígeno, antipiréticos, precauciones de aspiración y nutrición temprana.
  • Se observaron menos complicaciones, incluida la progresión del accidente cerebrovascular, infecciones en el pecho y deshidratación en el grupo de la unidad de accidente cerebrovascular.
  • La prevención de la aspiración, la alimentación temprana, la atención en la unidad de accidente cerebrovascular y la frecuencia de complicaciones influyeron de manera independiente en los resultados de los pacientes.

Conclusiones:

  • Existen diferencias significativas en el manejo y las tasas de complicaciones entre las unidades de accidente cerebrovascular y las salas generales, incluso con apoyo especializado.
  • Estas diferencias en el manejo y la atención probablemente explican los resultados más favorables observados en pacientes de la unidad de accidente cerebrovascular.
