The renin-aldosterone system is an endocrine system which guides the renal absorption of water and electrolytes, thus managing blood pressure and osmoregulation. Activation of the system begins in the kidneys with a small cluster of cells adjacent to the afferent and efferent blood vessels of the renal corpuscle. As the nephrons are filtering blood, juxtaglomerular cells monitor blood pressure. If they detect a decrease in pressure, they release the hormone renin into the bloodstream.

Sus antecedentes:

  • Las respuestas vasculares alteradas a los agonistas adrenérgicos son cruciales para la presión arterial y la regulación del flujo sanguíneo placentario durante el embarazo.
  • La vasoconstricción alfa1-adrenérgica y la vasodilatación beta2-adrenérgica se oponen entre sí para determinar el tono vascular.

Objetivo del estudio:

  • Para evaluar simultáneamente la vasoconstricción alfa-adrenérgica y la vasodilatación beta-adrenérgica en las venas dorsales de la mano durante y después del embarazo.
  • Comprender el papel de la sensibilidad adrenérgica en los cambios vasculares relacionados con el embarazo.

Principales métodos:

  • Estudió a 20 mujeres embarazadas sanas (32-37 semanas de gestación) y nuevamente 12 semanas después del parto.
  • Respuesta vascular medida a la fenilefrina (PE) y el isoproterenol (ISO) en las venas dorsales de la mano utilizando la técnica del transformador diferencial variable lineal.
  • Relaciones dosis-respuesta determinadas para la constricción inducida por la EP y la vasodilatación inducida por la ISO.

Principales resultados:

  • La sensibilidad alfa-adrenérgica (PE CD50) fue 7 veces menor durante el embarazo (2.7 μg/min) en comparación con el posparto (0.4 μg/min) (P<0.01).
  • La vasodilatación beta-adrenérgica se atenuó en el embarazo, con un ISO ED50 más alto (20 ng/min vs. 8 ng/min) y una respuesta máxima más baja (81% vs. 105%) (P<0.05 y P<0.01, respectivamente).

Conclusiones:

  • El embarazo normal exhibe una disminución de la sensibilidad venosa tanto a la vasoconstricción mediada por los alfa1-adrenorreceptores como a la vasodilatación mediada por los beta2-adrenorreceptores.
  • La mayor reducción en la respuesta alfa1-adrenérgica en comparación con la respuesta beta2-adrenérgica puede explicar el estado vasodilatado típico del embarazo humano.
