Sus antecedentes:

  • La luxación congénita de la cadera (CDH), ahora denominada displasia del desarrollo de la cadera (DDH), plantea riesgos significativos si se omite.
  • Los programas de detección tienen como objetivo la detección temprana, pero existen desafíos debido a la escasa comprensión de la maduración neonatal de la cadera.
  • La efectividad del cribado clínico y de ultrasonido para el DDH sigue siendo objeto de debate.

Objetivo del estudio:

  • Evaluar la efectividad de los programas de cribado universal versus selectivo para la displasia del desarrollo de la cadera.
  • Para comparar los resultados de diferentes estrategias de detección en recién nacidos.

Principales métodos:

  • Se llevó a cabo un ensayo aleatorio prospectivo que involucró a más de 15.500 recién nacidos.
  • El estudio comparó el cribado universal con programas de cribado selectivo.
  • Los resultados se evaluaron con un período de seguimiento de 6 a 11 años.

Principales resultados:

  • Se produjo un caso de displasia de cadera de detección tardía en el grupo de cribado universal, frente a seis en el grupo selectivo (no estadísticamente significativo).
  • No se encontró ningún beneficio adicional del cribado universal cuando ya existía un excelente programa clínico.
  • El programa de cribado universal en Coventry, Reino Unido, demostró resultados impresionantes y superiores en comparación con otros centros del Reino Unido.

Conclusiones:

  • Los objetivos del programa de cribado deben estar claramente definidos y rigurosamente evaluados.
  • Si bien el costo es un factor, el costo humano de los diagnósticos perdidos es inconmensurable.
  • El éxito del programa de detección universal de Coventry sugiere que podría servir como un punto de referencia para lograr mejores resultados en la detección de DDH en otros lugares.
