Resumen
La terapia génica localizada utilizando la sobreexpresión de HCN2 creó con éxito un marcapasos biológico en la aurícula izquierda canina. Este nuevo enfoque ofrece una alternativa prometedora para el tratamiento de enfermedades del marcapasos.

Sus antecedentes:
- El marcapasos natural del corazón se basa en los canales iónicos, incluidos los canales activados por hiperpolarización y por nucleótidos cíclicos (HCN).

Objetivo del estudio:
- Investigar el potencial de la sobreexpresión localizada de la isoforma HCN2 en la aurícula izquierda canina como un nuevo marcapasos biológico.
- Para evaluar la eficacia de la entrega del gen HCN2 en la restauración del ritmo cardíaco.
Principales métodos:
- Se inyectaron vectores adenovirales portadores de HCN2 de ratón o proteína fluorescente verde (GFP) en la aurícula izquierda canina.
- La corriente del marcapasos (I(f)) se midió en miocitos, y el ritmo cardíaco se monitoreó después de la detención sinusal inducida.
Principales resultados:
- Se observaron ritmos auriculares izquierdos espontáneos en 4 de cada 4 perros que recibieron HCN2 + GFP, en comparación con 0 de cada 3 perros que recibieron solo GFP.
- Se detectó una expresión significativa de la corriente del marcapasos (I(HCN2) en los miocitos de las aurículas tratadas con HCN2+GFP.
Conclusiones:
- La sobreexpresión localizada de HCN2 puede generar un marcapasos biológico funcional suficiente para impulsar el corazón.
- Este enfoque de terapia génica es prometedor para el tratamiento de la enfermedad del marcapasos y para reducir la dependencia de los dispositivos electrónicos.
