Resumen

El bajo peso al nacer perjudica la función endotelial al final del embarazo, aumentando el riesgo de enfermedad coronaria. El nacimiento muy prematuro puede mitigar este efecto, lo que sugiere que las interacciones entre el gen y el medio ambiente en el tercer trimestre son clave.

Sus antecedentes:

  • El bajo peso al nacer es un factor de riesgo conocido para la enfermedad coronaria en adultos.
  • La función endotelial vascular es crucial en las primeras etapas de la aterosclerosis.
  • Comprender el momento de la disfunción endotelial relacionada con el peso al nacer es importante para la intervención temprana.

Objetivo del estudio:

  • Para investigar el impacto del bajo peso al nacer en la función endotelial vascular en bebés.
  • Para determinar si la disfunción endotelial asociada con el bajo peso al nacer surge durante períodos gestacionales específicos.
  • Explorar la influencia del parto prematuro en la relación entre el peso al nacer y la función endotelial.

Principales métodos:

  • Se estudiaron 54 bebés a los 3 meses de edad postnatal, incluidos los nacimientos prematuros y a término.
  • Se midió la perfusión cutánea utilizando la técnica láser-Doppler.
  • Se evaluó la vasodilatación dependiente del endotelio a través de la iontoforesis de acetilcolina (ACh).

Principales resultados:

  • Los bebés nacidos a término y pequeños para la edad gestacional mostraron una función endotelial significativamente deteriorada en comparación con los controles de peso al nacer normales.
  • En los bebés muy prematuros, el peso al nacer (adecuado o bajo para la edad gestacional) no afectó significativamente la función endotelial.
  • La vasodilatación inducida por acetilcolina se redujo notablemente en los bebés nacidos a término pequeños para la edad gestacional.

Conclusiones:

  • La disfunción endotelial relacionada con el bajo peso al nacer se desarrolla o se hace evidente a finales del embarazo.
  • El parto muy prematuro parece atenuar la asociación negativa entre el bajo peso al nacer y la función endotelial.
  • Las interacciones entre los genes y el medio ambiente en el tercer trimestre pueden desempeñar un papel en estas diferencias observadas.
