Resumen
Las mujeres con neoplasia intraepitelial cervical de alto grado (CIN) tienen un mayor riesgo de neoplasia intraepitelial anal (AIN). La neoplasia genital multifocal aumenta significativamente el riesgo de involucramiento anal, lo que pone de relieve la necesidad de un cribado anal.

Sus antecedentes:
- Los carcinomas anogenitales invasivos están relacionados con el virus del papiloma humano (VPH) tipo 16.
- La neoplasia intraepitelial genital puede ser multifocal, pero la prevalencia de la neoplasia intraepitelial anal en estos casos está poco estudiada.
Objetivo del estudio:
- Para investigar la hipótesis de que las mujeres con neoplasia intraepitelial cervical de alto grado (CIN) tienen un mayor riesgo de neoplasia intraepitelial anal (AIN).
- Para determinar la prevalencia de AIN en mujeres con neoplasia intraepitelial del tracto genital.
Principales métodos:
- Examen histológico de biopsias anales en mujeres con neoplasia intraepitelial cervical grado III (n=152) en comparación con un grupo de control (n=50).
- Prueba de ADN del VPH 16 en muestras de biopsia anal.
- Evaluación de la neoplasia intraepitelial multifocal (cervix, vulva y vagina).
Principales resultados:
- El 19% de las mujeres con CIN III tenían evidencia histológica de AIN.
- El 57% de las mujeres con neoplasia intraepitelial genital multifocal tenían lesiones anales, en comparación con el 7% con solo lesiones cervicales.
- El ADN del VPH 16 se detectó en el 51% de las muestras de biopsia anal del grupo de estudio.
Conclusiones:
- Las mujeres con neoplasia intraepitelial cervical de alto grado tienen un mayor riesgo de neoplasia intraepitelial anal.
- La neoplasia intraepitelial genital multifocal está asociada con una prevalencia significativamente mayor de neoplasia intraepitelial anal.
- Se justifica una mayor investigación sobre el papel del examen anal en el manejo de las mujeres con neoplasia intraepitelial genital.
