Resumen
Es factible identificar los istmos de la taquicardia ventricular (VT) durante el ritmo sinusal utilizando el mapeo del ritmo. Esta técnica ayuda a delinear el curso del istmo VT y guía la ablación para mejorar los resultados del paciente.

- Los sitios de istmo fueron identificados por arrastre o terminación de VT, y se analizaron los datos de mapeo de ritmo.
- Se utilizó el mapeo de ritmo a diferentes distancias de la salida del istmo para trazar el curso del istmo.
Principales resultados:
- El mapeo de ritmo identificó partes de los istmos de la VT en las 13 regiones de infarto estudiadas.
- En la mayoría de los casos, el mapeo del ritmo produjo una morfología QRS que coincidía con VT.
- La ablación por radiofrecuencia guiada por mapeo de ritmo logró que la VT no fuera inducible en 10 de los 11 pacientes.
- Los sitios de ablación exitosos se ubicaron constantemente dentro de los istmos mapeados por ritmo.
Conclusiones:
- Los istmos VT pueden ser identificados y su curso delineado por la cartografía de ritmo durante el ritmo sinusal.
- Este método proporciona una estrategia valiosa para apuntar a los sitios de ablación.
- El mapeo del ritmo durante el ritmo sinusal puede ayudar a lograr una ablación exitosa del VT.
