Sus antecedentes:

  • La regurgitación aórtica (RA) presenta desafíos complejos debido a la patología aislada o combinada de la cúspide y la raíz.
  • Las estrategias de reparación sistemáticas e individualizadas son cruciales para la reconstrucción exitosa de la válvula aórtica.

Objetivo del estudio:

  • Evaluar la viabilidad y los resultados de un enfoque personalizado de reparación de la válvula aórtica para AR.
  • Evaluar la durabilidad y la morbilidad a medio plazo asociadas con la reparación individualizada de la válvula aórtica.

Principales métodos:

  • La reparación de la válvula aórtica se realizó en 282 pacientes con AR entre 1995 y 2003.
  • Las técnicas quirúrgicas incluían la reparación de la raíz (plicación, reemplazo, remodelación, reimplantación) y la reparación de la cúspide (plicación, resección, cierre del parche).
  • Se recopilaron datos de seguimiento para 8425 pacientes-meses, con una integridad del 99,6%.

Principales resultados:

  • La mortalidad hospitalaria fue del 3,9% (11 pacientes), y la reoperación por AR recurrente fue del 3,3% (9 pacientes).
  • La libertad de cinco años de grado de AR > o = II fue del 81% para la reparación de válvulas, del 84% para el reemplazo de raíz y del 94% para los procedimientos combinados.
  • La libertad de cinco años de reoperación fue de 93%, 95% y 98% para la reparación de válvulas, reemplazo de raíz y procedimientos combinados, respectivamente.

Conclusiones:

  • La reparación de la válvula aórtica es una opción viable para mecanismos complejos de AR utilizando una estrategia sistemática y personalizada.
  • El procedimiento demuestra una baja mortalidad operativa y una durabilidad a medio plazo alentadora.
  • La morbilidad relacionada con las válvulas es notablemente menor en comparación con el reemplazo de la válvula aórtica.
