An ischemic stroke occurs when a cerebral blood vessel becomes obstructed, most often by a thrombus or embolus, interrupting the delivery of oxygen and glucose to brain tissue. Sus antecedentes:

  • El accidente cerebrovascular grave a menudo conduce a la ventilación mecánica y a una alta mortalidad.

Objetivo del estudio:

  • Revisar la evidencia sobre el pronóstico para los pacientes con ACV con ventilación mecánica.
  • Establecer un marco para optimizar la toma de decisiones clínicas para pacientes y familias.

Principales métodos:

  • Búsqueda sistemática de la literatura de MEDLINE (1980-2005) para artículos relevantes en inglés.
  • Selección de 17 artículos de entre 689 identificados.
  • Análisis de los factores que influyen en las decisiones de tratamiento para mantener la vida y posibles sesgos.

Principales resultados:

  • Alta mortalidad general (aprox. 58% a los 30 días) en pacientes con ACV ventilados mecánicamente.
  • Los sobrevivientes pueden experimentar ninguna discapacidad / leve (hasta un tercio) o discapacidad severa.
  • El pronóstico se puede refinar utilizando el tipo de accidente cerebrovascular, las características del paciente y las reglas de predicción clínica.

Conclusiones:

  • El pronóstico es generalmente pobre, pero un subconjunto de pacientes sobrevive sin discapacidad grave.
  • La presentación clínica y las características del paciente ayudan en la evaluación del pronóstico.
  • Una mejor comprensión de las variaciones de la atención y la toma de decisiones centrada en el paciente es crucial.
