Sus antecedentes:

  • El ritmo apical ventricular derecho (AVR) puede conducir a una contracción ventricular izquierda anormal, hipertrofia y reducción de la función de la bomba.
  • La desincronización ventricular del ritmo RVA está relacionada con un mayor riesgo de hospitalización por insuficiencia cardíaca (HFH).

Objetivo del estudio:

  • Para identificar los predictores de HFH en pacientes sometidos a pacing de doble cámara (DDDR) frente al pacing ventricular (VVIR).
  • Para analizar el impacto de las características de referencia y los parámetros de ritmo en el riesgo de HFH.

Principales métodos:

  • El ensayo de selección de modo analizó a 2010 pacientes durante 6 años, comparando el ritmo DDDR y VVIR.
  • Los modelos de riesgos proporcionales de Cox se utilizaron para identificar los predictores de HFH, incluida la clase de la New York Heart Association (NYHA), la insuficiencia cardíaca, el bloqueo atrioventricular (AV), el infarto de miocardio (MI), el ritmo ventricular porcentual acumulado (Cum%VP) y la duración del QRS (QRSd).

Principales resultados:

  • La línea de base de la clase NYHA, la insuficiencia cardíaca, el bloqueo AV y el MI predijeron HFH.
  • Los predictores de postimplantación incluyeron un alto VVIR Cum%VP (>80%), umbrales específicos de DDDR/VVIR Cum%VP, y la duración del QRS de ritmo versus espontáneo.
  • La fracción de eyección baja (EF) y el QRSd más largo (especialmente cuando se acelera) fueron predictores significativos, con un aumento del riesgo más pronunciado en pacientes con EF normal.

Conclusiones:

  • El riesgo de HFH está influenciado por la interacción entre el sustrato del paciente (por ejemplo, fibrilación auricular, insuficiencia cardíaca, EF) y los parámetros de ritmo (por ejemplo, QRSd, Cum%VP, modo de ritmo).
  • El manejo efectivo del ritmo RVA es esencial para mitigar el riesgo de HFH, particularmente en grupos vulnerables de pacientes con EF baja e insuficiencia cardíaca existente.
