Sus antecedentes:

  • Existen preocupaciones con respecto a los efectos cancerígenos a largo plazo de la terapia con psoraleno y radiación ultravioleta A (PUVA).
  • PUVA es un tratamiento común para varios trastornos de la piel.

Objetivo del estudio:

  • Investigar la asociación entre la terapia con PUVA y el riesgo de desarrollar cánceres específicos.
  • Evaluar la relación dependiente de la dosis entre el tratamiento con PUVA y la incidencia de cáncer.

Principales métodos:

  • Un estudio de cohorte de 4799 pacientes suecos tratados con PUVA entre 1974 y 1985.
  • Análisis de la incidencia del cáncer en relación con el número de tratamientos PUVA recibidos y la demografía de los pacientes.
  • Los períodos de seguimiento promediaron 6.9 años para los hombres y 7.2 años para las mujeres.

Principales resultados:

  • Se observó un aumento significativo dependiente de la dosis en el riesgo de cáncer de células escamosas de la piel.
  • Los pacientes masculinos que recibieron más de 200 tratamientos PUVA mostraron una incidencia >30 veces mayor de cáncer de células escamosas en comparación con la población general.
  • También se observaron incidencias elevadas de cáncer respiratorio (hombres y mujeres), cáncer de páncreas (hombres) y cáncer de riñón y colon (mujeres) (p < 0,05).

Conclusiones:

  • El estudio confirma un aumento dependiente de la dosis en la incidencia de cáncer de células escamosas después de la terapia con PUVA.
  • La terapia con PUVA se asocia con un aumento de los riesgos para varios otros tipos de cáncer, lo que justifica una mayor investigación y una cuidadosa selección de pacientes.
