Resumen
El mesilato de imatinib mostró respuestas clínicas en pacientes con melanoma avanzado que albergaban alteraciones de KIT. Las respuestas fueron más probables en tumores con alteraciones KIT funcionalmente relevantes.

Sus antecedentes:
- Algunos melanomas, particularmente los de sitios acrales, mucosas y dañados por el sol, exhiben mutaciones activadoras y amplificación de la tirosina quinasa del receptor KIT.
- Este estudio investiga el potencial terapéutico de la inhibición de KIT utilizando imatinib mesilato en este subconjunto molecular específico del melanoma.
Objetivo del estudio:
- Para evaluar la eficacia clínica del mesilato de imatinib en pacientes diagnosticados con melanoma que albergan alteraciones de KIT.
- Evaluar la correlación entre las alteraciones moleculares específicas y la respuesta clínica al tratamiento con imatinib mesilato.
Principales métodos:
- Un ensayo de fase 2, abierto, de un solo grupo que involucró a 28 pacientes con melanoma no resecable avanzado y mutaciones o amplificación de KIT identificadas.
- Los pacientes recibieron imatinib mesilato en una dosis de 400 mg por vía oral dos veces al día.
- El objetivo primario fue la respuesta radiográfica, con objetivos secundarios que incluyen el tiempo hasta la progresión y la supervivencia general.
Principales resultados:
- Entre 25 pacientes evaluables, se observó una tasa de respuesta duradera del 16% (2 respuestas completas, 2 respuestas parciales duraderas).
- El tiempo medio hasta la progresión fue de 12 semanas, y la mediana de supervivencia global fue de 46,3 semanas.
- Se observó una mayor tasa de respuesta (40%) en tumores con mutaciones en puntos calientes recurrentes o una alta proporción de alelos mutantes a tipo salvaje, lo que sugiere una selección positiva.
Conclusiones:
- El mesilato de imatinib demuestra una actividad clínica significativa en un subconjunto de pacientes con melanoma avanzado con alteraciones de KIT.
- Las respuestas clínicas parecen estar asociadas con alteraciones de KIT de importancia funcional demostrada.
