Resumen
El sofosbuvir y la ribavirina trataron eficazmente el virus de la hepatitis C (VHC) en pacientes con coinfección por VIH, logrando altas tasas de respuesta virológica sostenida sin afectar negativamente la enfermedad o el tratamiento del VIH.

Sus antecedentes:
- El tratamiento del virus de la hepatitis C (VHC) en pacientes coinfectados por el virus de la inmunodeficiencia humana (VIH) es un desafío debido a las interacciones farmacológicas y las terapias basadas en interferón.
- Las opciones de tratamiento anteriores para la coinfección por el VHC y el VIH eran limitadas, lo que requería la exploración de regímenes sin interferón.
Objetivo del estudio:
- Evaluar la eficacia, medida por la respuesta virológica sostenida (RVS), de sofosbuvir y ribavirina en pacientes coinfectados con VHC y VIH.
- Evaluar la seguridad y los eventos adversos asociados con este régimen de tratamiento oral sin interferón en la población objetivo.
Principales métodos:
- Un ensayo abierto de fase 3 en el que participaron 223 pacientes con genotipos 1, 2 o 3 del VHC e infección simultánea por VIH.
- Los pacientes recibieron 12 o 24 semanas de sofosbuvir y ribavirina basada en el peso, dependiendo del genotipo del VHC y el historial de tratamiento.
- Para ser elegible se requiere terapia antirretroviral (TAR) estable para el VIH o el VIH no tratado con recuentos específicos de células T CD4 +.
Principales resultados:
- Se observaron altas tasas de SVR12 en todos los genotipos de VHC: 76% para el genotipo 1 (sin tratamiento), 88% para el genotipo 2 y 67% para el genotipo 3 (sin tratamiento).
- Los pacientes con experiencia en el tratamiento con genotipos 2 o 3 del VHC también mostraron altas tasas de SVR12 (92% y 94%, respectivamente).
- Los eventos adversos comunes incluyeron fatiga, insomnio y náuseas; solo el 3% interrumpió el tratamiento debido a eventos adversos, sin impacto en el estado de VIH.
Conclusiones:
- La combinación oral libre de interferón de sofosbuvir y ribavirina demuestra una alta eficacia para la erradicación del VHC en pacientes con coinfección por VHC-VIH.
- Este régimen ofrece una opción de tratamiento prometedora para individuos coinfectados por el VHC-VIH, lo que justifica una mayor investigación en diversas poblaciones.
- El tratamiento fue bien tolerado y no afectó negativamente la progresión o el manejo de la enfermedad por VIH.
