Resumen

La disfunción sistólica ventricular derecha (DVR) es común en pacientes con insuficiencia cardíaca con fracción de eyección preservada (HFpEF) y predice peores resultados, incluidas mayores tasas de mortalidad y hospitalización. Este hallazgo pone de relieve la importancia clínica de RVD en el manejo de HFpEF.

Sus antecedentes:

  • La prevalencia y la importancia de la disfunción sistólica ventricular derecha (DVR) en la insuficiencia cardíaca con fracción de eyección preservada (HFpEF) siguen sin estar claras.
  • HFpEF afecta a una población significativa de pacientes, lo que requiere una mejor comprensión de los factores contribuyentes y los indicadores de pronóstico.

Objetivo del estudio:

  • Para caracterizar la prevalencia y la importancia clínica de RVD en una cohorte HFpEF basada en la comunidad.
  • Para determinar la asociación de RVD con las características clínicas, los hallazgos ecocardiográficos y los resultados de los pacientes.

Principales métodos:

  • Identificación prospectiva de 562 pacientes HFpEF que cumplen con los criterios de Framingham (fracción de eyección ≥50%).
  • La ecocardiografía en el diagnóstico y el seguimiento para evaluar la función de RV utilizando la excursión sistólica del plano anular tricúspide y la evaluación 2D semicuantitativa.
  • Análisis de la mortalidad por causa específica y las tasas de hospitalización por HF, ajustadas por las covariables relevantes.

Principales resultados:

  • RVD se identificó en una proporción sustancial de pacientes con HFpEF, a menudo asociada con fibrilación auricular, marcapasos y uso de diuréticos.
  • Los pacientes con RVD exhibieron fracción de eyección ventricular inferior izquierda, peor disfunción diastólica, presión arterial más baja, mayor presión sistólica de la arteria pulmonar y mayor agrandamiento de RV y regurgitación tricúspide.
  • La RVD predijo de forma independiente una mayor mortalidad por todas las causas y cardiovascular, así como un aumento de las tasas de hospitalización por HF, independientemente de la definición utilizada (excursión sistólica en el plano anular semiquantitativa o tricúspide).

Conclusiones:

  • La disfunción sistólica ventricular derecha es un hallazgo frecuente en pacientes con HFpEF basados en la comunidad.
  • La RVD está vinculada a características de HF más avanzadas y presagia un pronóstico más pobre.
  • La evaluación de la función de RV es crucial para la estratificación del riesgo y la gestión de HFpEF.
