Sus antecedentes:

  • El acceso transradial (TRA) se prefiere al acceso transfemoral (TFA) para el cateterismo cardíaco debido a la reducción del sangrado y las complicaciones vasculares.
  • Sin embargo, existen preocupaciones con respecto a una exposición potencialmente mayor a la radiación con TRA.
  • La importancia clínica de esta diferencia de radiación requiere una evaluación.

Objetivo del estudio:

  • Para comparar la exposición a la radiación entre TRA y TFA para angiogramas coronarios de diagnóstico y intervenciones coronarias percutáneas (PCI).

Principales métodos:

  • Revisión sistemática y metanálisis de ensayos controlados aleatorios (ECA) y estudios observacionales.
  • Busqué en las bases de datos de PubMed, Embase y Cochrane Library.
  • Se centró en el tiempo de fluoroscopia y el producto de área de kerma; se analizó la dosis de radiación del operador y el tiempo del procedimiento como resultados secundarios.

Principales resultados:

  • La diferencia en el tiempo de fluoroscopia entre TRA y TFA ha disminuido significativamente en los últimos 20 años.

Conclusiones:

  • El acceso transradial está relacionado con un aumento menor de la exposición a la radiación en comparación con el acceso transfemoral.
  • Es poco probable que este aumento supere los beneficios clínicos establecidos de TRA.
  • La reducción de la brecha en la exposición a la radiación con el tiempo apoya aún más la utilidad clínica de TRA.
